Dragonfly/ Athletic Forms:
 
Every student MUST complete the following BEFORE trying out or playing for a McGill-Toolen Catholic team: 
  • Physical Form-must be completed by a physician and signed by BOTH the parent and the athlete. This is required yearly. Form attached below.
  • Sportsmanship Course (one time course): https://nfhslearn.com
  • Dragonfly- All forms are required and must be uploaded in the Dragonfly app. These forms must be completed yearly.  Dragonfly instructions attached below.
